The ten silent years

Type 2 diabetes does not arrive on the day the doctor says the word.

It has been building for years, quietly, while every report came back saying you were fine. And the reason it stayed hidden is simple, sister: they were measuring the wrong thing.

Here is what actually happens. You eat rice. Blood sugar rises. Your pancreas releases insulin to push it back down. Fine. But do that three times a day, for fifteen years, and your cells get tired of listening. So the pancreas has to shout louder. More insulin. Then more.

And your blood sugar reading stays normal the entire time, because the insulin is dragging it back into range by force. It looks like health. It is not health. It is a body working harder and harder to look fine.

Then one day the pancreas cannot shout any louder. The sugar climbs. And the doctor says the word.

Your blood sugar was never the first thing to go wrong. It was the last.

The test nobody offered you

Ask your GP for a fasting insulin test.

Not just glucose. Not just HbA1c. Fasting insulin, which tells you how hard your body is having to work to keep that glucose number looking normal.

This is the single most useful thing in this whole article, and it costs you nothing but the asking. Most sisters have never once been offered it, and it is the number that would have warned them a decade earlier.

Manage is not the same as heal

A tablet lowers the number on the meter. Alhamdulillah for medicine, and for the doctors who prescribe it, because it keeps people safe.

But be honest with yourself about what it is doing. It is managing the symptom. The plate that raised the sugar in the first place is still on the table.

That is why the dose so often goes up over the years, and almost never down. Nothing upstream has changed.

Change the plate, and you change the thing the medicine is fighting.

Why food is the lever

Insulin is released mostly in response to carbohydrate. Protein moves it a little. Fat barely moves it at all.

So when you take the rice, the kuih, the bihun and the teh manis off the plate, and you leave the rendang, the ikan, the sayur and the santan, you are not on a diet. You are removing the thing that was calling for the insulin in the first place.

Insulin comes down. And when insulin comes down, the body can finally start using its own fat stores instead of guarding them.

That is the mechanism. It is not magic, and it is not a promise. It is chemistry, and chemistry answers to what you feed it.

The honest picture

This is a mechanism, not a guarantee. Bodies are different, and Allah decides the outcome, not me and not a meal plan. What I can tell you is what I have witnessed in my kitchen, sister by sister, biiznillah. And what I will always tell you is to keep your doctor in the loop.

Where my lane ends and your doctor's begins

I want to be very clear, because this is where people get hurt.

Never stop your medication on your own. Never lower your own dose. Never do this quietly and hope for the best.

When you change how you eat, your blood sugar can drop, and if you are still on the same dose of medication designed to drop it further, that is dangerous. This is exactly why your doctor must be walking with you.

Tell them what you are doing. Ask them to monitor you. Let them adjust what needs adjusting. The kitchen walks beside your medical care. Never instead of it.

What you can do tonight

1. Ask for the fasting insulin test

At your next appointment. Say those three words. It is the number that tells the true story.

2. Take the rice off tonight's plate

Keep everything else exactly as it is. Why: rice becomes glucose in your blood within about ten minutes. The lauk does not.

3. Walk ten minutes after dinner

Why: working muscle clears glucose from the blood without needing much insulin. It is free, and it works from the very first evening.
